Some HMO plans require authorization. If your plan does require authorization, be sure to contact your primary care physician to have that authorization sent to us prior to your appointment. We must have your referral at the time of your appointment. You may be responsible for any balances, such as deductibles, copays, and any non-covered services (such as Refraction).As a courtesy to our patients, we will also submit claims to your supplemental insurance carrier. Any balances not paid by these insurances will be the responsibility of the patient.
Shaaf Eye Center offers financing through Care Credit. Please note this service subject to credit approval.
