Our physicians are participating with Medicare. We also participate in most insurance plans. A list of common insurances accepted at Shaaf Eye Center listed below. If you do not see your plan listed, please call our office directly at (760) 346-5005 and our team would be happy to verify your coverage.
We do not take EPMG, COV CA, Obama Care, or the Exchange.
Some HMO plans require authorization. If your plan does require authorization, be sure to contact your primary care physician to have that authorization sent to us prior to your appointment. We must have your referral at the time of your appointment. You may be responsible for any balances, such as deductibles, copays, and any non-covered services (such as Refraction).As a courtesy to our patients, we will also submit claims to your supplemental insurance carrier. Any balances not paid by these insurances will be the responsibility of the patient.
